What TSH, T3, T4 and thyroid antibodies mean

NHS laboratory guidance describes thyroid function tests as including TSH, free T4 and free T3. These values are interpreted as a pattern rather than as isolated numbers. View NHS thyroid function test information.

TSH: thyroid-stimulating hormone

TSH is produced by the pituitary gland and signals the thyroid to make thyroid hormones. A raised or suppressed TSH can be important, but the meaning depends on free T4, free T3, medicines, illness and clinical context.

Free T4: free thyroxine

T4 is the principal hormone released by the thyroid. Free T4 represents the unbound fraction available to tissues and is commonly considered with TSH when assessing possible underactive or overactive thyroid patterns.

Free T3: free triiodothyronine

T3 is the more biologically active thyroid hormone. Free T3 can be particularly useful in selected suspected hyperthyroid patterns, while illness and assay factors can complicate interpretation.

Thyroid peroxidase antibodies

TPO antibodies may support autoimmune thyroid context when included. NICE advises considering TPO antibodies in adults with TSH above the reference range and advises against routinely repeating them.

Thyroglobulin antibodies

Thyroglobulin antibodies are another marker of thyroid autoimmunity. Their presence does not, by itself, establish how well the thyroid is currently functioning.

Reference ranges

Laboratories use method-specific reference intervals. Age, pregnancy, medicines and assay methodology can alter how a result should be interpreted, so a number should not be compared casually with a range from another provider.

Why thyroid results must be read together

Raised TSH with low free T4This pattern can be consistent with primary hypothyroidism, but diagnosis and cause require clinical assessment.
Low TSH with raised free T4 or free T3This can be seen in hyperthyroidism, although medicines, pregnancy and assay interference can create misleading patterns.
